Terms and Conditions of the SCU Selects Program

• The Sharefax Credit Union (SCU) Select Employer Group (SEG) SCU Selects Rewards Program (“Selects”) is only available to specified employees who are SCU members within our SEGs as appointed by SCU.  At the sole discretion of SCU, you may be disqualified from the Selects if:

• You are in default under SCU Membership and/or Loan Agreements.

• You fail to meet the minimum performance standards set forth by SCU and agreed upon by both parties on your adoption of the Selects.

• You are no longer your company’s SCU Select, or

• For any other reason deemed appropriate by SCU.

Such disqualification will result in the loss of your accumulated points.

• The Selects program is offered at the sole discretion of SCU.  SCU may terminate the Selects or any portion thereof with 30 days prior notice to you.  However, SCU reserves the right to change or substitute rewards at any time without prior notification to you, and any rewards are subject to availability.  SCU may also change the point increment schedule at any time with 30 days prior notification to you.

• You are responsible for reporting and tracking your activities and points accrued.  Receiving SEG company approval of point accruing activities is your sole responsibility.  SCU is not responsible for consequences you may face as a result of any breach in SEG company policy.  SCU is not responsible for unreported or lost points.  Activities completed prior to January 1, 2008 do not qualify for points.  SCU may allow or disallow points at its sole discretion.

• Points may not be assigned, transferred, or pledged by any SCU Select to any third party.  As a SCU Select, if you leave your current position within your company and are no longer the SCU Select, or if you leave your company altogether, you may redeem your accrued points within 30 days of your last day as a SCU Select.  If you are terminated as a SCU Select by SCU, you may redeem accrued points within 30 days of your termination.  If you do not redeem them, you forfeit them.  However, if you become a SCU Select at another company within 90 days, you may rollover your points and continue to accrue points.

• For tax purposes, you will be required to complete a 1099 Miscellaneous Form for all reward compensation totaling over $600 within any calendar year.  SCU is not responsible for any consequences you may face as a result of not completing and filing this form.

• For rewards involving travel, the prize winner and guests will be required to travel together on the same itinerary and are responsible for obtaining all travel documents.  Travel is non-transferable.  Certain blackout periods and travel restrictions may apply.  SCU Selects must notify SCU of intended date of travel by no later than 60 days prior to the intended date of departure.

• “Airline tickets” include two roundtrip tickets to anywhere in the continental United States from a major airport near the SCU Selects residence.

• Rewards cannot be transferred by the SCU Selects or redeemed for cash.

• SCU Selects will assume all responsibility for any injury or damages caused or claimed to be caused by his or her participation in the SCU Selects Program and/or the acceptance and/or use of any SCU Selects Reward, and release Sharefax Credit Union, Inc. and its respective officers, directors, and employees from any such liability.

• SCU makes no guarantees, warranties, or representations of any kind concerning the Selects rewards.  SCU is not responsible for and does not warrant the condition of any reward.  SCU makes no representation or warranty concerning any reward or the reward’s fitness for purpose.  SCU has no liability in connection with the use or ownership of any reward and use of the reward is the sole responsibility of the SCU Select.  All Select rewards are subject to change as may be necessary to comply with State or local laws or regulations.  You release SCU from any kind of and all liability regarding the redemption or use of rewards.
